Rehabilitation Program
GLOVER TRANSITIONAL REHABILITATION UNIT
 All therapies and services are available 7 days a week. Grosvenor Park's staff work as one team. This team is comprised of your physician, nurses, therapists, discharge planner, dietician and, of course you and your family. This integrated approach to care recognizes you, as an individual with individual needs, goals and expectations, putting you at the core of the team.
Your team of rehabilitation specialists will guide you through each stepping stone of your recovery. The first step through the park will be a team evaluation to determine the goals of your stay. From this evaluation we will develop a specialized program designed just for you.
From the day of admission the Discharge Planner will work with you and the rest of the team to coordinate your return home. A home visit, if necessary, with your rehabilitation team will be completed to ensure a smooth and safe transition to home. The Discharge Planner will arrange for all community support services to begin upon your arrival home.
